knee replacement, also known as knee arthroplasty, is a surgical procedure to replace the weight-bearing surfaces of the knee joint to relieve pain and disability, most commonly offered when conservative treatments do not diminish joint pain. The main indications are osteoarthritis, rheumatoid arthritis/inflammatory arthritis, posttraumatic degenerative joint disease, or osteonecrosis/joint collapse with cartilage destruction.

Total knee arthroplasty (TKA) is associated with varying degrees of pain. A considerable proportion (25–40%) of patients experience severe pain, despite promising analgesic drugs.

The Anaesthesiologist Team (Vibrant) at Harmony hospital utilizes USG guided Adductor Canal Block along with multimodal analgesia technique to alleviate the surgical site pain of knee arthroplasty.

For years, femoral nerve block (FNB) has been considered as the main peripheral nerve block for postoperative analgesia following knee surgery. However, quadriceps weakness as the major downside of FNB led to searching for alternative nerve blocks. In recent years, adductor canal block (ACB) has been introduced as a pure sensory nerve block for postoperative analgesia following knee surgery .

How Block is Performed?

Volume Of Drugs Given

We use 0.25% bupivacaine 20 ml each side either with dexamethasone 8mg or clonidine 45microgram + Multimodal analgesia(MMA).

The effect of drugs usually lasts for 14-18 hours depending upon the drugs and volume used.

In conclusion, ACB is an emerging technique for postoperative analgesia following knee surgery with MMA and is as effective as FNB in postoperative pain control. The main advantage of ACB is preserving or minimal reduction in quadriceps strength that facilitates ambulation and rehabilitation after knee surgery. So far, more than 1000 total knee arthroplasty has been performed at Harmony Hospital and almost all patients were administered post-operative ACB along with MMA. Patients remained pain-free in the immediate postoperative period with early ambulation and pain-free stay at Harmony Hospital.
